All original paperwork for FR1s, FR2s, MR1s and Bankruptcy reinstatements must be sent in to be microfilmed in Drivers Services. Once all proper documents are in order, including clerks initials, Misc. Trans. Form #, and Field Office #, the office manager can make the update.

 

If the office manager does not have the access necessary to these screens, contact the appropriate update office and fax all documentation to them. Once the screens have been updated, the clerk can reinstate the license.
